CERTIFICATED STAFF FRINGE BENEFITS Group Health Insurance The percentage of premium contribution that shall be made for a regular employee shall be subject to the discretion of the Governing Board and amended from time to time. Effective July 1, 2008, the Governing Board contribution rate shall be as set forth below: A regular employee as defined by Policy GDBC whose first day of work was prior to July 1, 2008 shall be eligible for medical insurance and the Governing Board shall provide 80% of the monthly premium for either a single or family plan. Any such employee whose contract or corresponding work calendar is reduced effective after July 1, 2008 shall be subject to the eligibility and contribution rate in place for new employees as specified below. If an employee contract is decreased and approved prior to July, 1, 2008 but is not effective until after July 1, 2008, the following policy does not apply. A regular employee as defined by Policy GDBC whose first day of work was on or after July 1, 2008 shall be eligible for medical insurance and the Governing Board shall provide the following percentage of the monthly premium for either a single or family plan: Scheduled Hours per Week Contribution Rate 30 or more hours 80% 20 to 29 hours per week 60 Scheduled Work Days per Year Contribution Rate Minimum of four hours per day 160 or more days 80% 130 to 159 days 60% 110 to 129 days 50% If an employee is absent from work and not on an approved leave of absence (“dock” day), the employee shall be required to pay the daily prorated share of the premium paid by the Governing Board in accordance with procedures developed by the Administration and to the extent permitted by law. Enrollment in the HMO Plan sponsored by the Governing Board shall be closed for new enrollees effective the adoption date of this policy. Employees who have spouses who work in other districts belonging to the Clermont County Insurance Consortium (CCIC) shall be subject to the enrollment criteria and employer contribution rate established by CCIC. Dental Insurance The percentage of premium contribution that shall be made for a regular employee shall be subject to the discretion of the Governing Board as amended from time to time. Effective July 1, 2008, the Governing Board contribution rate shall be as set forth below. A regular employee as defined by Policy GDBC shall be eligible for dental insurance and the Governing Board shall provide 100% of the monthly premium for either a single or family plan. Vision Insurance The percentage of premium contribution that shall be made for a regular employee shall be subject to the discretion of the Governing Board as amended from time to time. Effective July 1, 2008, the Governing Board contribution rate shall be as set forth below: A regular employee as defined by Policy GDBC shall be eligible for vision insurance and the Governing Board shall provide 80% of the monthly premium for either a single or family plan.
